Medical Record Requests
Patients or authorized representatives may request copies of pre-hospital patient care reports where Montgomery EMS attended to the patient.
How to request records
To request the release of protected health information, such as a pre-hospital care report:

Complete the Authorization for Release of Health Information form.

Submit the completed form by:

Email to records@mems47.org, or
-
Mail (include a self-addressed stamped envelope for return mail).

Records cannot be released without an authorized signature.
Processing Time
While we make every effort to process requests promptly, please allow up to two weeks for records to be returned.

Paramedic records
In some emergencies, Mobile Intensive Care Unit (MICU) paramedics also respond and provide advanced medical care. When paramedics are involved, they generate a separate pre-hospital care report. These records must be requested directly from the paramedic organization.
